S. Pyogenes is PYR +, so are enterococci.
PYR + test is used for the rapid presumptive identification of streptococci.

Streptokinase -> transforms plaminogen to plasmin -> active proteolytic -> breaks down fibrin and other proteins -> breaks blood clots -> helps S. Pyogenes to escape from the blood clots. (Streptokinase is used for pulmonary emboli, coronary artery and venous thrombosis.)

Pyrogenic/antigenic exotoxin -> Streptococcal pyrogenic exotoxin -> Spe. 3 types. SpeA most studied -> superantigen -> binds to MHC II (V beta region) on T cells -> massive release of cytokines -> TSS + Scarlet Fever.

Cell wall has C Carb (Group A), M protein -> prevents complement activation and phagocytosis, however, B cells make antibodies against it -> opsonization -> phagocytosis.

G+, (Erysipelas, throat, skin, fasica, scarlet fever, toxic shock, heart, kidneys). PYR+, M proteins -> Rh fever, ASO titer, anti-DNAse B antibodies, JONES criteria, scarlet fever -> circumoral pallor.
